Hospital to Home Support Service for Seniors
The ILA Hospital to Home service continues for one to three months after your return home from hospital to help avoid a further crisis and the possible exhaustion of carers. Our expert Adviser will visit you regularly and there is also the option to continue ILA At Home support.

An example of how we help
Hospital to Home Support Service for Elders
Rita had been managing her existing medical condition and medication well on her own at home. However, following a fall at home Rita was admitted to hospital with a concussion and a fracture.
After five days in hospital Rita was medically fit to leave hospital but was very anxious about returning home. She has two
children who work full time and have families of their own but are not in a position to help on a regular basis. Rita's family contacted ILA for help with Rita's discharge and support in the early period of her recovery. ILA reviewed her situation with the hospital and organised an occupational therapist to visit her home. Minor adjustments, including handrails and non-slip mats were introduced to Rita's bathroom and furniture was rearranged to ensure future risks of slips or falls were minimised.
During Rita's period of recovery at home ILA identified she was not keeping on top of her washing and cleaning, her fridge was empty and her appetite had reduced. ILA worked with the family in coordinating food delivery and meal preparation and occasional cleaning.
Rita's confidence and strength improved and following the initial three months, Rita's family requested ILA stay in regular contact to prevent any future crisis.

ILA Hospital to Home Support Service costs from £1,000 to 2,350.
Private services introduced by ILA as part of your support plan will be contracted and paid for separately with prior agreement.
